Personalized license plates (often called custom or special license plates) allow Louisiana drivers to customize their vehicle’s license plate or recognize military service, affiliation, or causes. How to Get a Custom License Plate at Clearview Auto Title & Notary
Step 1: Know Before You Go
While requirements vary by plate type, most require:
- Valid photo ID
- Proper vehicle title and registration in your name in Louisiana.
- Certain specialty plates have specific qualifications and eligibility requirements (see Step 2).
- Personalized and specialty plates often include additional state-regulated fees. We’ll review costs with you during your visit.

Step 2: Gather the Required Documents
Handicap/Mobility Impaired License Plates
Only vehicles titled in Louisiana will be eligible for issuance of mobility impaired Plates.
Show Required Documents
1. A vehicle application form that our staff will complete with you.
2. A Physician's Certification of Mobility Impairment form completed by the mobility impaired individual's physician.
3. If the vehicle is titled in the mobility impaired person's name and is currently registered, the license plate and registration must be submitted.
4. If the plate is being used on a vehicle that is not in the name of the mobility impaired individual, our staff will help complete the Designation of Mobility Impaired License Plate form.
5. Proof of insurance.
6. If there is no license plate assigned to the vehicle at the time application for a mobility impaired plate is made, a copy of each vehicle owner’s current and valid photo identification (driver’s license or identification card issued by this state or another state) must be submitted.
Permanent Military Honors or Disabled Veteran License Plate
A qualified disabled veteran shall include any veteran who has a service-connected disability of fifty percent (50%) or more and who is a resident of this state.

1. You must be able to provide VA disability rating of 50% or more as assigned by the LDVA (find nearest location HERE).
2. Your Veterans Affair disability certification letter with your identifying information. Access your VA.gov login to find those letters.
3. A photocopy of the vehicle's registration certificate.
4. If the vehicle isn't registered in your name, the proper title documentation.
Military Veteran License Plate
You must be able to provide proof of military service with a valid and legible DD214 that notes the particular campaign and/or branch of service.

2. A photocopy of the vehicle's registration certificate.

First Responders License Plate

1. A letter from the chief official of your law enforcement agency stating that you have an active commission and have held it for at least two years.
2. Proof that you've completed Police Officer Standards Training (POST).
3. A photocopy of the vehicle's registration certificate if it's already registered.
4. If the vehicle isn't registered in your name, the proper title documentation.
Step 3: Visit Clearview to Complete the Transfer
Stop by one of our convenient locations, check in at the kiosk, and our team will prepare or review your custom license plate request.
Some plates are immediately available and other personalized plates can take approximately 3-5 months to process.
Louisiana Guidelines for Personalized License Plates

Remember, personalized license plates are issued on a first come, first serve basis.
