How Power of Attorney Notarization Works at Clearview

Step 1: Review the Power of Attorney Document

Bring the original POA document you need notarized. If you’re unsure whether it’s a general, limited, vehicle-specific, or special Power of Attorney, we’ll review it with you.


Power of Attorney documents are commonly used for:

Step 2: Bring Valid Identification

The person granting the authority (the principal) must present a current, acceptable photo ID.


Do not sign the document ahead of time unless it specifically instructs otherwise.


Some Power of Attorney documents require witnesses in addition to notarization. If you’re unsure, ask before visiting so you’re fully prepared.


For a Healthcare Directive or Medical Power of Attorney, Louisiana law requires two witnesses over age 18 who are not related to the signer, are not beneficiaries in the signer’s will, personally know the signer, and can confirm the signer is of sound mind.
