Act of Donation (In-State) Form
A Louisiana Parish Act of Donation form is a legally binding document used to formally transfer ownership of property—such as real estate, vehicles, or personal items—from one person (donor) to another (donee) as a gift, without any monetary exchange.

Vehicle Application Form 1799
The 1799 - Vehicle Application (DPSMV 1799) is an official form required by the Louisiana Office of Motor Vehicles (OMV) to process vehicle title and registration transactions. It collects vital information about the vehicle, owner, lienholder, and insurance to register, transfer, or title a vehicle.
The DPSMV 1966 form (Louisiana Medical Examiner's Certification of Mobility Impairment) is an official Louisiana document used to certify an individual's mobility impairment to obtain handicap parking placards or license plates. It must be completed by a licensed physician, chiropractor, or nurse practitioner, indicating if the impairment is temporary or permanent.
Form MCSA-5876 is the Medical Examiner's Certificate needed for Commercial Driver Medical Certification (CDL licenses).

Permission to Process Transaction Form DPSMV1806
The DPSMV1806 "Permission to Process Transaction" is a Louisiana Office of Motor Vehicles (OMV) form that authorizes a third party to handle specific vehicle-related tasks on behalf of the registered owner. This notarized document allows another person to manage renewals, transfers, or title work at an OMV or public tag agency.

Provisional Custody by Mandate Form
Provisional Custody by Mandate (PCBM) in Louisiana is a notarized document allowing parents to temporarily delegate care, custody, and control of a minor (under 18) to another adult for up to one year without court intervention.

Additional Form of Identification
An extra document used to support identity verification, such as a Social Security card or secondary ID, when required.
Affidavit
A written statement that a person swears is true and signs in front of a notary. Affidavits are commonly used for legal, personal, or vehicle-related matters.
Authorized OMV Service Provider
A private business approved by the Louisiana Office of Motor Vehicles to process certain OMV services, such as licenses, titles, registrations, and plates.

Bill of Sale
A document showing the sale and purchase details of a vehicle, including price and vehicle description. In some situations, it must be notarized.
C -
Certified / True Copy
A copy of a document that a notary certifies as a true and accurate reproduction of the original, when allowed by law.
Class A Driver’s License
A commercial driver’s license required to operate combination vehicles with a high gross vehicle weight rating, such as tractor-trailers.
Class B Driver’s License
A commercial driver’s license required to operate single large vehicles, such as buses or straight trucks.
Class C Driver’s License
A license that allows operation of standard passenger vehicles. In some contexts, it may also apply to certain commercial vehicles not requiring Class A or B.
Class “E” License
The standard Louisiana driver’s license that allows unrestricted driving once all graduated licensing requirements are met.
Commercial Driver’s License (CDL)
A special driver’s license required to operate commercial vehicles such as large trucks or buses. Additional testing and requirements apply.
Custom License Plate
Another term commonly used for a personalized or specialty license plate. It may refer to plates with custom lettering or special designations.

Driver Education Provider
A state-approved school or organization authorized to deliver classroom and behind-the-wheel driver education instruction.
Duplicate Title
A replacement vehicle title issued when the original title has been lost, stolen, or damaged.

Eligibility Requirements
Specific qualifications that must be met to receive certain licenses, plates, permits, or services, such as age, service status, or documentation.
Endorsement
A special authorization added to a driver’s license allowing the holder to operate certain vehicles or carry specific cargo.

Restrictions or alerts placed on a driver’s license or record that prevent certain transactions until the issue is resolved.
G -
Graduated Driver’s License (GDL)
Louisiana’s step-by-step licensing system for young drivers that includes a Learner’s Permit, Intermediate License, and full Class “E” license.
H -
A placard that allows eligible individuals to park in designated handicap-accessible spaces. Medical certification is required.
Hold
A restriction placed on a driver’s record that prevents certain services from being completed until the issue is resolved.
I -
Identification Card
A state-issued photo ID for individuals who do not hold a driver’s license. It does not grant driving privileges.
Identity
Verification that a person is who they claim to be, typically established using official government-issued documents.
Intermediate License
A restricted driver’s license issued to drivers, typically age 16, who have completed the Learner’s Permit phase but are still subject to driving limitations.

Knowledge (Written) Driving Test
A written or computer-based test that evaluates knowledge of traffic laws, signs, and safe driving practices.
L -
Lawful Status
Proof that a person is legally authorized to be in the United States, as required for certain identification and licensing services.
A permit that allows a new driver to practice driving while supervised by a qualified, licensed driver.
Lien
A legal claim placed on a vehicle by a lender until a loan is paid in full.
Limited Term Driver's License
A limited-term driver’s license is a U.S. government-compliant ID or driver's license issued to foreign nationals with temporary lawful status. These licenses have an expiration date matching the authorized stay, often maximum of five years, and are used when legal presence documents expire in less than 5 years. While they have the same driving privileges, they are marked to signify temporary status. Clearview Auto Title & Notary DO NOT issue Limited Term Licenses or Renewals.

Mobility-Impaired Hang Tag / License Plate
A placard or license plate issued to individuals with qualifying mobility impairments, allowing access to designated parking spaces.
N -
No Insurance (NI) Flag
A specific flag placed on a vehicle or driver record when required insurance coverage has lapsed or cannot be verified.
Notary / Notarization
A notary is a licensed official who verifies identity, witnesses signatures, and notarizes documents to help prevent fraud.
O -
Odometer Disclosure Statement
A statement confirming a vehicle’s mileage at the time of sale or transfer, required for most non-exempt vehicles.
OMV (Office of Motor Vehicles)
The Louisiana state agency responsible for driver’s licenses, vehicle titles, registrations, and license plates.
OMV Approved Public Tag Agent
A private business authorized by the Louisiana Office of Motor Vehicles (OMV) to process certain services such as titles, registrations, license plates, and select driver’s license transactions.
Original Birth Certificate
An official birth certificate issued by a state or local government. Photocopies or unofficial versions are typically not accepted.
P -
A license plate that allows the vehicle owner to choose specific letters or numbers, subject to availability and state approval.
Photo Retrieval Program / Database
An OMV system used to access and verify an existing driver’s license or ID photo on file when issuing or updating credentials.
A legal document that allows one person to authorize another to act on their behalf for specific matters. It must be properly notarized to be valid.
Primary Document
An official document used to prove identity or lawful status, such as a birth certificate or passport.
Principal Residence
The primary address where a person lives. Proof of principal residence is required for many licensing and ID services.
Proof of Insurance
Documentation showing active vehicle insurance coverage that meets Louisiana’s minimum requirements.
Provisional Custody by Mandate
A notarized document that allows a parent or legal guardian to temporarily grant caregiving authority to another adult.
R -
A federally compliant driver’s license or ID required for domestic air travel and entry into certain federal facilities beginning May 7, 2025.
REAL ID Act
A federal law establishing security standards for state-issued IDs and driver’s licenses. A REAL ID-compliant credential is required for domestic air travel and access to certain federal facilities beginning May 7, 2025.
Reinstatements
The process of restoring driving privileges after a license has been suspended, canceled, or otherwise restricted, once all requirements are met.
Registration
The process of registering a vehicle with the state so it can be legally driven on public roads.
Remote Online Notary (RON)
A method of notarization performed electronically using approved technology, allowing parties to notarize documents remotely.
Road Skills Test
A practical driving test that evaluates a driver’s ability to safely operate a vehicle on public roads.

Secondary Document
A supporting document used to verify identity, residency, or other information when combined with a primary document.
Special / Specialty License Plate
A license plate issued for a specific cause, organization, service recognition, or affiliation, often with additional eligibility requirements.
Suspension
A temporary removal of driving privileges due to violations, non-compliance, or legal issues.
T -
Temporary Instructional Permit (TIP)
A permit required for students enrolled in driver education, allowing classroom instruction and supervised driving during training.
A legal document that establishes ownership of a vehicle.
The process of changing vehicle ownership from one person to another, whether through sale, donation, or inheritance.
U -
UCC-1 (Financing Statement)
A document filed to record a lender’s security interest in a vehicle when a lien is placed.
Used Title Transfer
The process of transferring ownership of a used vehicle from one private owner to another.
V -
Vehicle Application Form
A required form used to apply for title, registration, or related vehicle services, typically completed during processing.
Vehicle Registration Renewal
The process of extending a vehicle’s registration so it remains legal to operate on public roads.
VIN (Vehicle Identification Number)
A unique identification number assigned to a vehicle, used to verify vehicle history and ownership.
Vision Exam
An eyesight test required to confirm a driver meets minimum vision standards for safe operation of a vehicle.

Witness
A person who observes the signing of a document when required, in addition to notarization.
